IAPT - Wednesdays 9:00-17:00
Croydon IAPT is a free and confidential NHS service provided by the South London and Maudsley NHS Foundation Trust.
They offer a range of psychological therapies to adults 18 years and over, who are registered with a Croydon GP.
IAPT (Improving Access to Psychological Therapies) is a national programme that was created in 2005 after it was recognised that a very large percentage of the population suffer with problems like depression and anxiety at some point in their lives. Each county and London borough have their own local IAPT service that provides evidence based treatment for common problems like these.
Live Well - 10:00-12:00 Wednesday's & Friday 14:00-16:00 PM
WHAT IS JUST LIVE WELL?
Just Live Well is a 12 month, face-to-face service delivered by qualified advisors, which works with Croydon residents who might find it difficult to make changes in their life without additional support.
It is designed to provide support that can help you:
• quit smoking
• lose weight
• eat more healthily
• become more physically active
• cut down on alcohol
• improve mental wellbeing
The programme uses recognised coaching techniques such as motivational interviewing and goal-setting to help you make meaningful changes to your life

Social Prescribing -Every other Thursday- 11am-15:00
Social prescribing, sometimes referred to as community referral, is a means of enabling GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services.
Social prescribing schemes can involve a variety of activities which are typically provided by voluntary and community sector organisations. Examples include:
- Arts activities
- Group learning
- Healthy eating advice
- A range of sports
Welfare benefits Advisor- Wednesday's 14:00-16:00 PM
Experienced benefits advisor offers one-to-one meetings, where you can discuss your circumstances and the advisor can identify possible entitlement to all available welfare benefits, tax credits, health benefits, or other services provided by the local authority or the voluntary sector.
We invite all patients to join our Patient Participation Group.
Our Patient Participation Group is a partnership between the practice and its patients, we meet regularly with our practice leaders and GPs to discuss matters relevant to you, as well as local and national issues.

Are you a carer?
Here at Eversley we hold carers meetings. We offer support. In our meetings you will meet other members of the group that may face the same daily challenges. We also provide useful information from our council, and have a list of organisation that can help.

Connecting your care
What information will be shared?
The information shared will include:
- Name, address and NHS number
- Appointments and hospital attendances
- Important medical history, including diagnoses, medicines and allergies
- Test results, such as blood tests and x-rays
What information will not be shared?
Certain information is classified as highly sensitive and is restricted by law, such as sexual health, assisted conception and adoption records and this information will not be shared.
Is my information safe?
Information about you is protected under the Data Protection Act 2018 and the General Data Protection Regulations 2016. This requires that health and social care professionals:
- Can see information about you only so that they can provide you with the care that you need
- Must keep your records secure
- Have a duty of confidence
In addition, all organisations are governed by the General Data Protection Regulation 2016, which means they are responsible for your information and must keep it safe.
Information about you will not be shared with or sold to private companies.
Who will see the information about me?
Only the people directly involved in your care, at the time and place when you see them, or when they are preparing to see you or following up results, clinical letters or follow up care. This might be your GP or practice nurse, hospital doctors and nurses, along with administrators who might be arranging your appointments and care.
